In the companies I have helped convert we promoted learning and improving by

Setting aside time
to select measures of success
to discuss problems
to discuss and select solutions
to discuss the effectiveness of changes made
to using measurements to promote exchanges and communication
to use measurements to show people what is important to the business and
guide actions
to know one another
Rerquiring
measurements of action take before and after the action
require improvment in the process used
focus on the process not the person to promote communication

It is simple it just requires the discipline to remember the mission is to
improve the process not just run hard to serve the customer.
