like myself and anyone else interested in trying to build successful
teams:

Team Traps: Survival Storms and Lessons from Team Disasters,
Near-Misses, Mishaps and other Near Death Experiences By Steven R.
Raynor

and

No More Teams! Mastering the Dynamics of Creative Collaboration, By
Micheal Schrage

The later is excellent, pointing at more dynamic alternatives to teams.
