It appears that you are asking for a system to manage knowledge in a
specific domain, ie. "lessons learned" in the course of a project.

It would seem that you need a text management system (not a relational
database!) with a built-in querying capability. There are several of
these on the market - Basis from Information Dimensions, and Fulcrum from
the company of the same name. Once you've selected the software, then you
can select the hardware.

An alternative to text management software is "Help Desk" software. This
type of software is used to support help desks. One of it's functions is
to record problems and solutions, and provide access to them.

The key question is how do you want to be able to access the information?
This will determine how the information should be stored.
