I saw the model not in a learning-organization or general business book
but, surprisingly, in a book about the mental side of the game of golf.
Dr. Bob Rotella, a renowned american sports psychologist, introduces it
step by step in his golf teaching book "Golf is not a game of perfect"
and I also think in his second book "golf is game of confidence". I don4t
know if this is of any help to you.

Best regards
