While I haven't read it yet, an Amazon.com reviewer of the recently
released book by Dee Hock "Birth of the Choardic Age" suggests that Hock
speaks directly to this issue. Hock is the creator of the VISA card. He
has also provides some good advise about leadership in an article for Fast
Company http://www.fastcompany.com/online/05/dee2.html

This is a pressing issue in the healthcare industry particularly around
the "mission" of HMOs.
