I checked out the earlier reference to a 1962 book by Dubin and that
wasn't it. (It was, however, a dandy book full of first-rate readings in
management.) In case you're interested, the citation is...

I also followed up with John Gunkler who offered to check with Larry
Wilson to see if Larry knew the source. Larry recollected hearing about
it at a conference in Buffalo, something to do with an Institute for
Creative Education. I take that to be the Creative Education Foundation,
home to creative problem solving. I have quite a collection of materials
from that, which I promptly checked. No luck.
So, the source of the conscious-unconscious, competent- incompetent model
remains a mystery--for now.
