David inquires as to the origins of the learning model that runs from
unconscious incompetence to unconscious competence (passing through
conscious incompetence and conscious competence along the way).

I believe it's from Neuro-Linguistic Programming (NLP) by Bandler and
Grinder, but I can't say for sure. It is cited a lot without giving any
credit or hint to its origins. Whether NLP is the source or not, NLP
certainly makes use of the concepts in a big way so that should be of some
help.
