Another thought for "grading' team performance comes in a different
package. We call it the "Reflection" step, and use a plus/delta format.

Every problem-solving and process planning team, consisting of never more
that eight individuals, must present their work at least at the time of
completion. This Quality Improvement Story takes about 15 minutes and
allows every team member to participate . One key element is the
Plus/Delta feedback which the team provides itself and the audience
composed of team sponsor, facilitator, managers and co-workers. The "plus"
consists of several key, positive attributes to the team's just-completed
work. The "delt a," using the mathematical symbol for change, describes
one or more key features of the just completed team work which would be
changed next time to improve the way teams work.

For about two years our company culture has been adopting (and continues
to adopt, and adapt to) the Plus/Delta format. It's used not only for team
presentations as a form of self-analysis, but also for team meetings (or
any meetings for that matter) as a simple method to focus on process,
among other things.
