OL Qualitative Questionairre LO19030

I am putting together a list of question prompts for series of semi
structured interviews to try to guage to what extent a company is a L.O.
I have used Senge's Five Disciplines as headers and constructed questions
below that level. Are there any templates about that I can do a sanity
check against?
